    Validity evidence of the organizational justice scale in Spain

    Resumen tomado de la publicaciónEvidencias de validez de la Escala de Justicia Organizacional en España. Antecedentes: el interés por la medición de la percepción de justicia organizacional ha aumentado en los últimos años debido a su demostrada relación con resultados organizacionales significativos, como el bienestar y el agotamiento emocional. En España, se puede destacar la Escala de Justicia Organizacional (OJS) como un instrumento que ha mostrado buenas propiedades psicométricas en estudios previos en el sector hotelero. Este estudio complementa la evidencia de las características de la OJS utilizando una amplia muestra multisectorial. Método: los participantes fueron 849 empleados de diferentes sectores ocupacionales. La estructura de la OJS se estudió mediante análisis factorial exploratorio y confirmatorio dividiendo la muestra en dos submuestras aleatorias. Además, se analizó la fiabilidad y validez de cada dimensión. Resultados: los resultados indicaron que la escala está constituida por tres factores (distributiva, procedimental y en la interacción), con una fiabilidad y validez adecuadas. Por otra parte, como era de esperar, se encontraron correlaciones positivas entre la percepción de justicia organizacional y el bienestar, y negativas con el agotamiento emocional. Conclusiones: la OJS es una herramienta adecuada para su uso por parte de académicos y profesionales en el estudio de la percepción de justicia organizacional en España, garantizando una validez y fiabilidad adecuadas.Universidad de Oviedo.     Effectiveness of an Invasive Mechanical Ventilation Weaning Protocol

    Objective: to know the effectiveness of the process of disconnection of invasive mechanical ventilation through the implementation of a standardized protocol led by the nursing team of a surgical Intensive Care Unit and implemented in a multidisciplinary way to reduce the ventilatory time of patients facing to the traditional clinical method of weaning. Method: case-control study. 91 patients who required invasive mechanical ventilation for 24 hours or more were included and a group with the application of the standardized protocol was compared with the group that followed the usual clinical method. Results: with the application of the standardized protocol, it was possible to reduce the time of invasive mechanical ventilation (85.29 ± 46.72 vs 116.92 ± 94.39); the time spent at weaning (2.40 ± 1.43 vs 41.250± 51.60) and the reintubation figures (2% vs 17%). Conclusion: the use of disconnection protocols carried out in a multidisciplinary way is a useful tool to reduce invasive mechanical ventilation times and achieve improvements in the patient’s health

    Diagnosis of Subclinical Keratoconus Based on Machine Learning Techniques

    Background: Keratoconus is a non-inflammatory corneal disease characterized by gradual thinning of the stroma, resulting in irreversible visual quality and quantity decline. Early detection of keratoconus and subsequent prevention of possible risks are crucial factors in its progression. Random forest is a machine learning technique for classification based on the construction of thousands of decision trees. The aim of this study was to use the random forest technique in the classification and prediction of subclinical keratoconus, considering the metrics proposed by Pentacam and Corvis. (2) Methods: The design was a retrospective cross-sectional study. A total of 81 eyes of 81 patients were enrolled: sixty-one eyes with healthy corneas and twenty patients with subclinical keratoconus (SCKC): This initial stage includes patients with the following conditions: (1) minor topographic signs of keratoconus and suspicious topographic findings (mild asymmetric bow tie, with or without deviation; (2) average K (mean corneal curvature) 490 μm; (4) no slit lamp found; and (5) contralateral clinical keratoconus of the eye. Pentacam topographic and Corvis biomechanical variables were collected. Decision tree and random forest were used as machine learning techniques for classifications. Random forest performed a ranking of the most critical variables in classification. (3) Results: The essential variable was SP A1 (stiffness parameter A1), followed by A2 time, posterior coma 0°, A2 velocity and peak distance. The model efficiently predicted all patients with subclinical keratoconus (Sp = 93%) and was also a good model for classifying healthy cases (Sen = 86%). The overall accuracy rate of the model was 89%. (4) Conclusions: The random forest model was a good model for classifying subclinical keratoconus. The SP A1 variable was the most critical determinant in classifying and identifying subclinical keratoconus, followed by A2 time

    Diminuição do tempo ventilatório mediante protocolo de desconexão multidisciplinar. Estudo piloto

    Objective: compare ventilatory time between patients with the application of a disconnection protocol, managed in a coordinated way between doctor and nurse, with patients managed exclusively by the doctor. Method: experimental pilot study before and after. Twenty-five patients requiring invasive mechanical ventilation for 24 hours or more were included, and the protocol-guided group was compared with the protocol-free group managed according to usual practice. Results: by means of the multidisciplinary protocol, the time of invasive mechanical ventilation was reduced (141.94 ± 114.50 vs 113.18 ± 55.14; overall decrease of almost 29 hours), the time spent on weaning (24 hours vs 7.40 hours) and the numbers of reintubation (13% vs 0%) in comparison with the group in which the nurse did not participate. The time to weaning was shorter in the retrospective cohort (2 days vs. 5 days), as was the hospital stay (7 days vs. 9 days). Conclusion: the use of a multidisciplinary protocol reduces the duration of weaning, the total time of invasive mechanical ventilation and reintubations. The more active role of the nurse is a fundamental tool to obtain better results.Objetivo: comparar el tiempo ventilatorio entre pacientes sometidos a desconexión según un protocolo manejado de forma coordinada por el médico y la enfermera con el mismo tiempo en pacientes manejados exclusivamente por el médico. Método: estudio piloto experimental antes y después. Se incluyeron a 25 pacientes que requirieron ventilación mecánica invasiva durante 24 horas o más, y se comparó el grupo orientado por protocolo con el grupo sin protocolo, manejado según la práctica habitual. Resultados: mediante el protocolo multidisciplinar se logró disminuir el tiempo de ventilación mecánica invasiva (141,94 ± 114,50 vs. 113,18 ± 55,14; disminución global de casi 29 horas), el tiempo empleado en el destete (24 horas vs. 7,40 horas) y las cifras de reintubación (13 % vs. 0%) en comparación con el grupo en el que no participó la enfermera. El tiempo hasta iniciar el destete fue menor en la cohorte retrospectiva (2 días vs. 5 días), así como también la estancia hospitalaria (7 días vs. 9 días). Conclusión: la adopción de un protocolo multidisciplinar disminuye la duración del destete, el tiempo total de ventilación mecánica invasiva y las reintubaciones. El papel más activo de la enfermera se considera una herramienta fundamental para obtener mejoras en los resultados.Objetivo: comparar o tempo ventilatório em pacientes submetidos a desconexão segundo um protocolo conduzido de forma coordenada pelo médico e pela enfermeira com o mesmo tempo em pacientes tratados exclusivamente pelo médico. Método: estudo piloto experimental antes e depois. Foram incluídos 25 pacientes que precisaram de ventilação mecânica invasiva durante 24 horas ou mais, e o grupo orientado por protocolo foi comparado com o grupo sem protocolo, tratado conforme a prática habitual. Resultados: mediante o protocolo multidisciplinar, conseguiu-se reduzir o tempo de ventilação mecânica invasiva (141,94 ± 114,50 vs. 113,18 ± 55,14; redução global de quase 29 horas), o tempo empregado no desmame (24 horas vs. 7,40 horas) e as cifras de reintubação (13% vs. 0%) em comparação com o grupo em que não houve participação da enfermeira. O tempo até iniciar o desmame foi menor na coorte retrospectiva (2 dias vs. 5 dias), bem como a internação hospitalar (7 dias vs. 9 dias). Conclusão: a adoção de um protocolo multidisciplinar diminui a duração do desmame, o tempo total de ventilação mecânica invasiva e as reintubações. O papel mais ativo da enfermeira é considerado uma ferramenta fundamental para obter melhorias nos resultados

    Pharmacogenetic polymorphisms affecting bisoprolol response

    This article is part of the thesis: Analysis of genetic variants associated with response to bisoprolol in patients with acute coronary syndrome with percutaneous coronary intervention with stent, within the doctoral program in Pharmacy at the University of Granada, to whom we thank for their collaboration. Kontula for sharing with us detailed information from their articles [16,22] which have allowed us to perform the meta-analysis.β-blockers are commonly prescribed to treat multiple cardiovascular (CV) diseases, but, frequently, adverse drug reactions and intolerance limit their use in clinical practice. Interindividual variability in response to β-blockers may be explained by genetic differences. In fact, pharmacogenetic interactions for some of these drugs have been widely studied, such as metoprolol. But studies that explore genetic variants affecting bisoprolol response are inconclusive, limited or confusing because of mixed results with other β-Blockers, different genetic polymorphisms observed, endpoint studied etc. Because of this, we performed a systematic review in order to find relevant genetic variants affecting bisoprolol response. We have found genetic polymorphism in several genes, but most of the studies focused in ADRB variants. The ADRB1 Arg389Gly (rs1801253) was the most studied genetic polymorphism and it seems to influence the response to bisoprolol, although studies are inconclusive. Even, we performed a meta-analysis about its influence on systolic/diastolic blood pressure in patients treated with bisoprolol, but this did not show statistically significant results. In conclusion, many genetic polymorphisms have been assessed about their influence on patients´ response to bisoprolol and the ADRB1 Arg389Gly (rs1801253) seems the most relevant genetic polymorphism in this regard but results have not been confirmed with a meta-analysis. Our results support the need of further studies about the impact of genetic variants on bisoprolol response, considering different genetic polymorphisms and conducting single and multiple SNPs analysis, including other clinical parameters related to bisoprolol response in a multivariate study

    Vigilancia virológica de Aedes (Stegomyia) aegypti y Aedes (Stegomyia) albopictus como apoyo para la adopción de decisiones en el control del dengue en Medellín

    Introduction: Dengue represents an important public health problem in Colombia. No treatment is available and the vaccine has not been approved in all countries, hence, actions should be strengthened to mitigate its impact through the control of Aedes aegypti, the vector mosquito. In Colombia, surveillance is done using entomological indexes and case notification, which is usually informed late, leading to untimely interventions. Viral detection in urban mosquitoes using molecular techniques provides more accurate entomological information for decision-making.Objective: To report results of virological surveillance in Aedes specimens collected during routine entomological activities of the Secretaría de Salud de Medellín.Materials and methods: Specimens were collected during two periods in each of which we selected 18 dwellings around each one of the 250 larva traps arranged for mosquitoe surveillance, as well as 70 educational institutions and 30 health centers. Specimens were identified morphologically, and divided in pools for viral detection using reverse transcription polymerase chain reaction (RT-PCR). We calculated the minimum infection rate and the adult infestation index for each group.Results: We collected 1,507 adult mosquitoes, 10 of which were identified as A. albopictus. Out of the 407 pools, 132 (one of them Ae. albopictus) were positive, and 14.39% were A. aegypti males. The minimum infection rates for Ae. aegypti were 120.07 and 69,50 for the first and second periods, respectively, and the adult infestation index was higher in educational institutions (23.57%).Conclusions: Using RT-PCR we identified natural infectivity and vertical transmission of dengue virus in A. aegypti and A. albopictus. We suggest the use of molecular techniques in arbovirosis surveillance and control programs in Colombia.Introducción. El dengue en Colombia representa un grave problema de salud y, dado que no existe un tratamiento efectivo para la enfermedad y la vacuna no se ha aprobado en todos los países, se deben fortalecer acciones para mitigar su impacto mediante el control de Aedes aegypti, el mosquito vector. La vigilancia en el país se hace con base en los índices entomológicos y en la notificación de casos, la cual es frecuentemente tardía y por ello conduce a falta de oportunidad en las intervenciones. La detección viral en mosquitos urbanos mediante técnicas moleculares proporciona información entomológica más precisa para la adopción de decisiones.Objetivo. Reportar los resultados de la vigilancia virológica de especímenes de Aedes spp. recolectados durante actividades entomológicas rutinarias de la Secretaría de Salud de Medellín.Materiales y métodos. Los ejemplares se recolectaron durante dos periodos, en cada uno de los cuales se seleccionaron 18 viviendas alrededor de cada una de las 250 trampas para larvas dispuestas para la vigilancia entomológica, así como 70 instituciones educativas y 30 centros de salud. Los ejemplares se identificaron y se conformaron grupos para la detección viral mediante reacción en cadena de la polimerasa con transcripción inversa (Reverse Transcription Polymerase Chain Reaction, RT-PCR). Se calculó la tasa mínima de infección y el índice de infestación en adultos.Resultados. Se recolectaron 1.507 mosquitos, 10 de los cuales eran Ae. albopictus. De los 407 grupos conformados, 132 (uno de ellos de Ae. albopictus) fueron positivos, y 14,39 % correspondió a machos de Ae. aegypti. La tasa mínima de infección para Ae. aegypti fue de 120,07 y 69,50 en el primer y segundo períodos, respectivamente, y el índice de infestación en adultos fue mayor en las instituciones educativas (23,57 %).Conclusión. Mediante la RT-PCR se detectaron la infección natural y la transmisión vertical del virus del dengue en Ae. aegypti y en Ae. albopictus.     Immunogenic dynamics and SARS-CoV-2 variant neutralisation of the heterologous ChAdOx1-S/BNT162b2 vaccination: Secondary analysis of the randomised CombiVacS study

    Background The CombiVacS study was designed to assess immunogenicity and reactogenicity of the heterologous ChAdOx1-S/BNT162b2 combination, and 14-day results showed a strong immune response. The present secondary analysis addresses the evolution of humoral and cellular response up to day 180. Methods Between April 24 and 30, 2021, 676 adults primed with ChAdOx1-S were enrolled in five hospitals in Spain, and randomised to receive BNT162b2 as second dose (interventional group [IG]) or no vaccine (control group [CG]). Individuals from CG received BNT162b2 as second dose and also on day 28, as planned based on favourable results on day 14. Humoral immunogenicity, measured by immunoassay for SARS-CoV-2 receptor binding domain (RBD), antibody functionality using pseudovirus neutralisation assays for the reference (G614), Alpha, Beta, Delta, and Omicron variants, as well as cellular immune response using interferon-γ and IL-2 immunoassays were assessed at day 28 after BNT162b2 in both groups, at day 90 (planned only in the interventional group) and at day 180 (laboratory data cut-off on Nov 19, 2021). This study was registered with EudraCT (2021-001978-37) and ClinicalTrials.gov (NCT04860739). Findings In this secondary analysis, 664 individuals (441 from IG and 223 from CG) were included. At day 28 post vaccine, geometric mean titres (GMT) of RBD antibodies were 5616·91 BAU/mL (95% CI 5296·49–5956·71) in the IG and 7298·22 BAU/mL (6739·41–7903·37) in the CG (p 1:100 at day 180 (19% and 22%, respectively). Interpretation Titres of RBD antibodies decay over time, similar to homologous regimes. Our findings suggested that delaying administration of the second dose did not have a detrimental effect after vaccination and may have improved the response obtained. Lower neutralisation was observed against Omicron and Beta variants at day 180.Funded by Instituto de Salud Carlos III (ISCIII).     Muscle Recovery after a Single Bout of Functional Fitness Training

    Background: Functional fitness training (FFT) is a new exercise modality that targets functional multi-joint actions via both muscle-strengthening exercises and aerobic training intervals. The aim of the study was to examine muscle recovery over a 20 min period after an FFT workout in trained adults. Materials and methods: Participants were 28 healthy trained subjects. In a single session, a countermovement jump (CMJ) was performed to determine several mechanical variables (jump height, maximum velocity, power) before (preFFT) and 4, 10, and 20 min after the FFT workout (postFFT). In parallel, capillary blood lactate concentrations were measured pre- and 3 min postFFT. Heart rate was also measured before and after the workout, and perceived exertion was measured postFFT. Results: Significant differences between the time points preFFT and 4 min and 10 min postFFT, respectively, were produced in jump height (p = 0.022, p = 0.034), maximum velocity (p = 0.016, p = 0.005), average power relative (p = 0.018, p = 0.049), and average power total (p = 0.025, p = 0.049). No differences were observed in any of the variables recorded preFFT and 20 min postFFT. Conclusions: While mechanical variables indicating muscle fatigue were reduced 4 and 10 min postFFT, pre-exercise jump ability only really started to recover 20 min after FFT although not reaching pre-exercise levels.     Five Patients with Disorders of Calcium Metabolism Presented with GCM2 Gene Variants

    The GCM2 gene encodes a transcription factor predominantly expressed in parathyroid cells that is known to be critical for development, proliferation and maintenance of the parathyroid cells. A cohort of 127 Spanish patients with a disorder of calcium metabolism were screened for mutations by Next-Generation Sequencing (NGS). A targeted panel for disorders of calcium and phosphorus metabolism was designed to include 65 genes associated with these disorders. We observed two variants of uncertain significance (p.(Ser487Phe) and p.Asn315Asp), one likely pathogenic (p.Val382Met) and one benign variant (p.Ala393_Gln395dup) in the GCM2 gene in the heterozygous state in five families (two index cases had hypocalcemia and hypoparathyroidism, respectively, and three index cases had primary hyperparathyroidism). Our study shows the utility of NGS in unravelling the genetic origin of some disorders of the calcium and phosphorus metabolism, and confirms the GCM2 gene as an important element for the maintenance of calcium homeostasis.     Association of occupational heat exposure and colorectal cancer in the MCC-Spain study

    Objective Heat exposure and heat stress/strain is a concern for many workers. There is increasing interest in potential chronic health effects of occupational heat exposure, including cancer risk. We examined potential associations of occupational heat exposure and colorectal cancer (CRC) risk in a large Spanish multi-case- control study.Methods We analyzed data on 1198 histologically confirmed CRC cases and 2690 frequency-matched controls. The Spanish job-exposure matrix, MatEmEsp, was used to assign heat exposure estimates to the lifetime occupa-tions of participants. Three exposure indices were assessed: ever versus never exposed, cumulative exposure and duration (years). We estimated odds ratios (OR) and 95% confidence intervals (CI) using unconditional logistic regression adjusting for potential confounders.Results Overall, there was no association of ever, compared with never, occupational heat exposure and CRC (OR 1.09, 95% CI 0.92-1.29). There were also no associations observed according to categories of cumulative exposure or duration, and there was no evidence for a trend. There was no clear association of ever occupational heat exposure and CRC in analysis conducted among either men or women when analyzed separately. Positive associations were observed among women in the highest categories of cumulative exposure (OR 1.81, 95% CI 1.09-3.03) and duration (OR 2.89, 95% CI 1.50-5.59) as well as some evidence for a trend (P<0.05).Conclusion Overall, this study provides no clear evidence for an association between occupational heat exposure and CRC
